Sending a  patient Transition of Care document to another Healthcare entity Multiple users are able to successfully send a patient CCDA record out of the practice to Regional Health Care exchange and/or other providers of patient care that can accept CCDA files.   Each user in the testing needs to receive at least one CCDA file per user and display the information.  Obtain average time to perform task and test each CCDA file in the validator for errors or issues. (b)(1) – Transitions of Care Sending to either another provider outside of the practice or to a Regional Health Exchance is a common and important data integration task. Outpatient Pediatrics Perform this task at least 3x per year with the testing group.    Anticipate that over 95% can perform task in less than 5 minutes and over 98% pass CCDA validation Three tests in year.  Recommend Apr, Jul, Sept April, July, Sept 2022
Receiving a patient record, link to a patient and incorporate the relative data into the patient record. Multiple users are able to succesfully receive CCDA information on an existing or new patient and incorporate this into the clinical record for the patient.  Each user in the test should reconcile and Incorporate at least one element such as demographic, medication, problem or allergy into the clinical record for appropriate patient.  Measure average time to complete the task as well as verify the information was linked to the clinical record for the patient selected. (b)(2) – Clinical Information Reconciliation and Incorporation Receiving patient data from either another provider outside of the practice or from a Hospital is a common and important data integration task. Outpatient Pediatrics Perform this task at least 3x per year with the testing group.    Anticipate that over 95% can perform task in less than 5 minutes and over 90% pass CCDA validation Three tests in year.  Recommend Apr, Jul, Sept April, July, Sept 2022
Export patient CCDA files for select patients of a time range. Multiple users are able to successfully export patients seen at the practice for a select time frame.  Measure time to complete the task and validate the CCDA file format. (b)(6) – Data Export Some pediatric practices need to be able to export patient CCDA records for various uses. Outpatient Pediatrics Perform this task at least 3x per year with the testing group.    Anticipate that over 95% can perform task in less than 5 minutes (if less than 100 patients) and over 98% pass CCDA validation Three tests in year.  Recommend Apr, Jul, Sept April, July, Sept 2022
Run a Clinical Quality Measure (CQM) report and export the data Multiple users show ability to run CQM report for a select time frame as well as export the data associated the report.  Measure time to complete the task and calculate average time. (c)(1) – Clinical Quality Measures – Record and Export The CQM reports are a tool related to value based agreements that Pediatric Practices might participate Outpatient Pediatrics Perform this task at least 1x per year with the testing group.    Anticipate that over 95% can perform task in less than 10 minutes Test during Summer of 2022 July 2022
Run a Clinical Quality Measure (CQM) report with QRDA detailed data. Users are able to run a CQM report and export the QRDA data for all or some of the patients selected.  Record time to complete activity and confirm QRDA file meets requirements (c)(3) – Clinical Quality Measures – Report Some CQM reporting function might need the QRDA patient data for submission with the summary reports Outpatient Pediatrics Perform this task at least 1x per year with the testing group.    Anticipate that over 95% can perform task in less than 15 minutes Test during Summer of 2022 July 2022
Verify a trained user can operate API tool to generate a patient or list of patients Users follow the API instructions and tools to test the API interface using the postman tool. (g)(7) – Application Access – Patient Selection The API tool provides some flexibility for Pediatric practices to interface potentially with different tools and organizations. Outpatient Pediatrics Perform this task at least 1x per year with the testing group.    Anticipate that over 95% can perform task in less than 15 minutes and user obtains accurate patient information over 90% of tries. Test during Summer of 2022 July 2022
Verify that the API tool generates patient data in at least 5 categories Users follow the API instructions to request data on at least 5 values per patient.   Measure time to complete tasks. (g)(8) – Application Access – Data Category Request A trained user should be able to pull out select API data Outpatient Pediatrics Perform this task at least 1x per year with the testing group.    Anticipate that over 95% can perform task in less than 15 minutes and user obtains accurate patient information over 90% of tries. Test during Summer of 2022 July 2022

EMR Direct Interoperability Engine is utilized to send the CCDA files. (b)(1) – Transitions of Care EMR Direct Interoperability Engine Sending to either another provider outside of the practice or to a Regional Health Exchance is a common and important data integration task. Outpatient Pediatrics Perform this task at least 3x per year with the testing group.    Anticipate that over 95% can perform task in less than 5 minutes and over 98% pass CCDA validation Three tests in year.  Recommend Apr, Jul, Sept April, July, Sept 2024 TBD in 2024
Receiving a patient record, link to a patient and incorporate the relative data into the patient record. (b)(2) – Clinical Information Reconciliation and Incorporation NA Receiving patient data from either another provider outside of the practice or from a Hospital is a common and important data integration task. Outpatient Pediatrics Perform this task at least 3x per year with the testing group.    Anticipate that over 95% can perform task in less than 5 minutes and over 90% pass CCDA validation Three tests in year.  Recommend Apr, Jul, Sept April, July, Sept 2024 TBD in 2024
Verify a trained user can export electronic health information for one patient record and the patient population Authorize Users follows the instructions and tools to export electronic health information for one patient and for the patient population (b)(10) – Electronic Health Information Export NA The ability to export electronic health information for a patient or the patient population is limited to the administrative users Outpatient Pediatrics Perform this task at least 1x per year with the testing group.    Anticipate that over 95% can perform task in less than 15 minutes and user obtains accurate patient information over 90% of tries. Test during Summer of 2024 July 2024 TBD in 2024
Run a Clinical Quality Measure (CQM) report and export the data Multiple users show ability to run CQM report for a select time frame as well as export the data associated the report.  Measure time to complete the task and calculate average time. (c)(1) – Clinical Quality Measures – Record and Export NA The CQM reports are a tool related to value based agreements that Pediatric Practices might participate Outpatient Pediatrics Perform this task at least 1x per year with the testing group.    Anticipate that over 95% can perform task in less than 10 minutes Test during Summer of 2024 July 2024 TBD in 2024
Run a Clinical Quality Measure (CQM) report with QRDA detailed data. Users are able to run a CQM report and export the QRDA data for all or some of the patients selected.  Record time to complete activity and confirm QRDA file meets requirements (c)(3) – Clinical Quality Measures – Report NA Some CQM reporting function might need the QRDA patient data for submission with the summary reports Outpatient Pediatrics Perform this task at least 1x per year with the testing group.    Anticipate that over 95% can perform task in less than 15 minutes Test during Summer of 2024 July 2024 TBD in 2024
Verify a trained user can operate API tool to generate a patient or list of patients Users follow the API instructions and tools to test the API interface using the postman tool. (g)(7) – Application Access – Patient Selection NA The API tool provides some flexibility for Pediatric practices to interface potentially with different tools and organizations. Outpatient Pediatrics Perform this task at least 1x per year with the testing group.    Anticipate that over 95% can perform task in less than 15 minutes and user obtains accurate patient information over 90% of tries. Test during Summer of 2024 July 2024 TBD in 2024
Verify that the API tool generates patient data in all categories. Users follow the API instructions to request data on all the available categories.   Measure time to complete tasks.  Test data for completeness. (g)(9) – Application Access – All Data Request NA A trained user should be able to link all available data to the API data export Outpatient Pediatrics Perform this task at least 1x per year with the testing group.    Anticipate that over 95% can perform task in less than 15 minutes and user obtains accurate patient information over 90% of tries. Test during Summer of 2024 July 2024 TBD in 2024
